HOM Cares is here to help connect friends and family with the day to day visits of their loved ones in Hospice of Michigan care. Even those interested in simply learning more about us can find up-to-date reading material and send feedback directly to Hospice of Michigan before ever logging in. After securely logging in, detailed patient visit information can then viewed in chronological order, and scrolled through. Pictures and description of your loved ones hospice-based visitors are also available with one tap right from the visits list. Those who hold Primary Caregiver status have the ability to grant access, as well as remove access to other friends and family members, allowing them to login and view visits too.
